To refer a patient, you may: 

• Call, text, or fax us the patient's name, date of birth, phone number, and reason for the referral

What to expect following a referral to A Plus Eyes: ​

• We will contact the patient to get them scheduled with us.

• We will report back to your office with our findings and recommendations after seeing the patient.

• The patient will be instructed to return to your office to fill any optical prescriptions given.​
